These Terms contain important clauses regarding medical disclaimers, liability limitations, clinician verification, and your responsibilities as a healthcare professional. By using PraxisMD you agree to be bound by these Terms in their entirety.
By accessing or using the PraxisMD application ("the App", "Software"), you ("User", "Clinician", "Student") agree to be bound by these Terms & Conditions ("Terms") in their entirety. If you do not agree to these Terms, you must not use the App.
These Terms constitute a legally binding agreement between you and PraxisMD ("we", "us", "our"). Use of the App indicates unconditional acceptance of these Terms. We reserve the right to update these Terms at any time; continued use following notification of changes constitutes acceptance of the revised Terms.
PraxisMD IS A CLINICAL DOCUMENTATION TOOL ONLY. It is not a medical device, diagnostic instrument, or clinical decision-support system, and must not be used as such.
The App does not provide medical advice, diagnoses, treatment recommendations, or prescriptions. All clinical decisions — including but not limited to diagnoses, investigations, prescriptions, referrals, and management plans — are the sole professional responsibility of the licensed clinician using the App.
No content generated, suggested, or displayed by the App should replace professional clinical judgement. PraxisMD expressly disclaims any liability for clinical outcomes arising from the use of this App.
Students and trainees must ensure that all documentation created using this App is reviewed, supervised, and countersigned by an appropriately qualified and registered supervising physician before it is used for any clinical purpose.
All clinical notes, patient records, annotations, diagnoses, prescriptions, and any other data you enter into the App ("User Content") are and remain your sole property. PraxisMD claims no ownership over User Content.
BY USING THE APP, YOU ACCEPT FULL AND SOLE RESPONSIBILITY FOR:
User Content is stored locally on your device and/or your chosen cloud storage folder (OneDrive, iCloud Drive, Google Drive, or equivalent). PraxisMD does not transmit, access, or retain your clinical content on any PraxisMD-controlled server.
The App is licensed, not sold. Subject to these Terms, PraxisMD grants you a limited, non-exclusive, non-transferable licence to use the App for lawful clinical documentation purposes in accordance with your selected plan (Clinician or Student).

The Student / Teaching Mode is intended solely for clinical students and trainees for educational documentation practice under appropriate clinical supervision.
Documentation created in Teaching Mode must NOT be used as official clinical records, prescriptions, or any other document with direct patient-care implications, unless reviewed, corrected, and countersigned by a registered supervising clinician.
The supervising physician named within the App accepts no responsibility by virtue of being listed unless they have independently reviewed and signed the documentation in question.
In Teaching Mode, patient-identifiable information is automatically anonymised in PDF exports as a safeguard. Users remain responsible for ensuring no identifiable data is inadvertently entered or exported.
Clinician accounts are subject to verification of professional registration. By registering as a Clinician, you:
PraxisMD reserves the right to request additional verification at any time. Full access is granted during the verification review period. Verified status may be revoked if credentials are found to be invalid, expired, or fraudulent.
Your account is identified by a unique anonymous identifier stored in your browser's local storage. You are responsible for:
If you lose access to your device, you may use your recovery code to reset your PIN. PraxisMD cannot recover accounts on your behalf as we do not store passwords or PINs on our servers.
Your account exists within your browser's local storage. If you clear your browser data or switch devices without migrating your data, you may lose access to your account and data. PraxisMD is not liable for data loss resulting from browser data clearance.
A one-time licence fee valid for two (2) years from the date of purchase. Renewal at $9.99/year thereafter. No automatic recurring charges.
An annual subscription at the rate displayed at the time of purchase. A 7-day free trial is offered to new users; you will be charged the annual fee at the end of the trial period unless you cancel before it expires.
All payments are processed by Stripe, Inc. By making a payment you also agree to Stripe's Terms of Service. PraxisMD does not store your payment card details.
All fees are exclusive of applicable taxes. Refunds are provided only where required by applicable consumer law or at PraxisMD's sole discretion. PraxisMD reserves the right to change pricing with reasonable notice; price changes will not affect your current paid subscription period.
Initiating a chargeback without first contacting PraxisMD support constitutes a breach of these Terms and may result in immediate account suspension.
All rights, title, and interest in and to the App — including its source code, design, user interface, trademarks, logos, and documentation — are the exclusive property of PraxisMD and are protected by applicable intellectual property laws.
You acquire no intellectual property rights in or to the App by virtue of these Terms or your use of the App. Feedback, suggestions, or ideas you provide regarding the App may be used by PraxisMD without restriction or compensation to you.
Where you enter identifiable personal data (including patient demographic information), you act as the data controller under applicable data protection law. You are solely responsible for obtaining all necessary consents, complying with data subject rights, notifying any data breaches, and ensuring the security of the data you store.
We collect and process your name, email address, registration number, and verification documents for the purpose of account management and clinician verification. Payment data is handled exclusively by Stripe. See our Privacy Policy for full details.
We use PostHog for privacy-first analytics to understand how the App is used. Analytics do not include patient data. You may opt out at any time in App Settings.
The FHIR export feature converts your clinical notes into HL7 FHIR R4 format for interoperability. FHIR exports are generated locally on your device. PraxisMD does not receive or process FHIR data.
TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW:
Nothing in these Terms shall limit liability for fraud, wilful misconduct, or any other liability which cannot be excluded by law.
You agree to indemnify, defend, and hold harmless PraxisMD, its affiliates, officers, directors, employees, and agents from and against any and all claims, liabilities, damages, losses, costs, and expenses (including reasonable legal fees) arising out of or in any way connected with:
PraxisMD reserves the right to suspend or terminate your access to the App without notice if you breach these Terms, provide false credentials during verification, or if your use of the App poses a risk to patient safety, professional standards, or the integrity of the App.
You may terminate your use at any time by ceasing to use the App and cancelling your subscription. Termination does not entitle you to a refund of any fees paid except where required by applicable consumer law.
These Terms are governed by and construed in accordance with applicable law in the jurisdiction in which the App is used. Any dispute arising out of or in connection with these Terms that cannot be resolved by good-faith negotiation shall be referred to the appropriate courts or alternative dispute resolution mechanisms of the relevant jurisdiction.
If any provision of these Terms is found to be unenforceable, the remaining provisions shall continue in full force and effect.
